NEW PATIENT FORMS
Save time by completing these forms ahead of time. Bring them completed along with your insurance card to your initial visit with us.
PRESCRIPTION REFILLS
When calling for a prescription refill, we will need the name of the medication, the dosage and your pharmacy’s phone number. Please allow 48 hours to process prescription refill requests.
As a general policy, we do not request refills after hours or on the weekends.
FINANCIAL POLICY
All co-payments, deductibles and non-covered services should be paid at the time of service.
For your convenience, we accept cash, checks, debit and credit cards.
